Telephone Assessment

​​​​​​​​​​​​​​​​​​​​​​Contact with patients by telephone is a common part of nursing practice. Both the Oncology and Haematology Services have their own guidelines for nursing staff working within those services on the requirements for taking telephone calls.

  • Symptom prompt sheets are available here and in each clinical area to guide nursing staff on questions to ask and provide a flag system to escalate review as required.

  • Currently two forms exist to document telephone calls - the Telephone Triage Form (C240282) and Chemotherapy Follow Up Form (C240281).

  • These two forms are ordered through the supply department and are never photocopied.

  • Telephone calls which are patient/family initiated should be documented on the Telephone Triage Form.

  • Routine nurse initiated telephone calls to assess patient tolerance to chemotherapy are documented on the Chemotherapy Follow Up Form.
