Telephone Assessment

​​​​​​​​​​​​​​​​​​​​​​Contact with patients by telephone is a common part of nursing practice. Both the Oncology and Haematology Services have their own guidelines for nursing staff working within those services on the requirements for taking telephone calls.

  • Symptom prompt sheets are available here and in each clinical area to guide nursing staff on questions to ask and provide a flag system to escalate review as required.

  • Currently documenting telephone calls occurs on the Telephone Triage Assessment and Planned Treatment Follow up Form (C240282).

  • This form is ordered through the supply department and are never photocopied.
