Contact with patients by telephone is a common part of nursing practice. Both the Oncology and Haematology Services have their own guidelines for nursing staff working within those services on the requirements for taking telephone calls.
Symptom prompt sheets are available here and in each clinical area to guide nursing staff on questions to ask and provide a flag system to escalate review as required.
Currently two forms exist to document telephone calls - the Telephone Triage Form (C240282) and Chemotherapy Follow Up Form (C240281).
These two forms are ordered through the supply department and are never photocopied.
Telephone calls which are patient/family initiated should be documented on the Telephone Triage Form.
Routine nurse initiated telephone calls to assess patient tolerance to chemotherapy are documented on the Chemotherapy Follow Up Form.
