An acceptable neurological state exists: ie the patient is awake enough to protect and maintain their airway.
Assisted spontaneous breathing with pressure support of 5cm H2O
The spontaneous respiratory rate is less than 30 breaths per minute
The patient is stable with on FiO2 40%o with PaO2 60mmHg and/or SpO2 90%
PEEP is between 5 and 10cm H2O
Haemodynamic stability exists
There is no prospect of a major intervention planned in the ensuing 24 hours.
