The Community Services within the Older Persons Health Service comprises of six interdisciplinary Community Services teams that cover the greater Christchurch area.

The teams are comprised of the following services:

  • Adult Community Therapy Service
  • Community Rehabilitation Enablement & Support Team (CREST)
  • Community Stroke Rehabilitation Service
  • North East and South West Community Teams
  • Older Persons Mental Health Community Team
