Enquiries and Feedback sent from this form are NOT MONITORED over the Christmas / New Year period.

  • Older Persons Health & Rehabilitation (OPH&R) and Community Dental services correspondence will not be monitored from 5pm Friday 18 December 2020.
  • Christchurch Hospital Services and Specialist Mental Health Services correspondence will not be monitored from 4pm Thursday 24 December 2020.

Normal monitoring for correspondence/feedback for all services listed above resumes at 8am on Tuesday 5 January 2021.

For patients and/or their families enquiring about care and treatment: Please contact the hospital concerned by telephone or in person and ask for the Nurse in Charge of the ward in the first instance, or alternatively you can ask for the Duty Manager.
