​​​​​If you have received a letter from the Diabetes Centre advising you have an appointment, please email us or call us to confirm (or change) your appointment using the contact details below:

To confirm or change your appointment, please:

Please remember when emailing or leaving a message to include your:

  • full name

  • national health identifier

  • the day and time of your appointment.

  • a contact telephone number ​and the best time for us to return your call


On the day of your appointment, please bring:

  • your appointment letter

  • your glucose meter and blood glucose record book (if you have one)

  • food record sheet, and

  • a list of your current medications (or bring your medications).

It is important that you confirm your appointment, or contact us if this appointment is no longer required so that we can make it available to other patients.
