Please remember, limiting our interactions with others is our best defence against COVID-19. Please don’t visit the hospital if you don’t need to. The following level 2 visitor restrictions are in place for all Canterbury DHB health facilities:

  • Visitors will be screened and asked to provide contact details upon arrival
  • Visitors who are unwell or have suspicion of COVID-19 will not be able to enter
  • Aged residential care: no visitors, however, family visits for palliative care residents who do not have COVID-19 will be considered on a case-by-case basis.

Change your contact details

Options to change contact information held by Canterbury DHB

You can you change your contact details:

  1. When visiting your GP Practice.
  2. By telephoning the department or service you are under the care of.
  3. By asking the receptionist when attending an appointment.
  4. Completing a paper form at Christchurch Hospital main reception.
  5. Online – you can use our online form to request a change to your:
    • Name
    • Postal or residential address
    • Contact telephone number
    • The name of your GP/Doctor

      A separate online form application is currently required for each person changing their details, including other family members.
