The following visitor restrictions are in place for all Canterbury DHB hospitals and health facilities:

Registering your project sets in motion a request for services. There are 5 options:

The Research Office need to have received an email from the Service /Line Manager with approval of the requested FTE by this date in order for approvals to be signed off in time.
Please fill out this form if you are solely applying for Locality Authorisation to undertake research using CDHB resources eg. Non CDHB hosted clinical trial.
If you believe that your project should meet the criteria for a ultra low or low risk locality application please fill out the questions below. If not, please submit the form.
You must be able to tick ALL the boxes to qualify as a low risk project.
