Each year DHBs are required to release information relating to serious adverse events that have occurred within their hospitals. A serious adverse event (SAE) is one that has resulted in significant additional treatment, major loss of function, is life-threatening or has led to an unexpected death.
The DHB is required to review these events and report them to the Health, Quality & Safety Commission. The Commission then collates this information and it is published on their website www.hqsc.govt.nz
